1. Refund & Cancellation Policy

Strict No-Refund Policy: 212 Lacrosse operates under a “No Refund Policy” for all programs, teams, and events. This applies to all circumstances, including but not limited to injuries, illnesses, or weather-related cancellations.

Registration Insurance: To protect your investment, we highly recommend the “Registration Saver” insurance from AIG, available during registration. This is the only mechanism for recovering fees due to unforeseen circumstances.

Non-Refundable Deposits: All deposits are strictly non-refundable and serve to secure a player’s spot in the program.

Private Training: A 48-hour cancellation policy applies to all private sessions. Sessions canceled within 48 hours of the scheduled start time will be charged in full.
2. Payment Authorization & Auto-Billing
By registering for any 212 Lacrosse program, you authorize 212 Lacrosse to maintain a secure payment method on file. This payment method will be used as the primary or backup billing source for all services within the 212 Lacrosse ecosystem. This includes:

Private & Group Instruction: Automated billing for training sessions.

Outstanding Arrears: Any unpaid balances from current or previous seasons.

Ancillary Charges: Late cancellation fees, apparel, equipment, or travel fees.

Administrative Efficiency: This system ensures our staff remains focused on elite player development rather than manual collections.
3. Rate Integrity & Outstanding Balances
At 212 Lacrosse, we provide premium, professional services. To maintain the integrity of our program and fairness to all 250+ families:

Fixed Rates: All rates are non-negotiable. Requesting discounts after services have been rendered is inconsistent with our professional standards and will not be entertained.

Prompt Payment: Payment is due at the time of registration or immediately following a private session.

Late Fees: Any balance outstanding for more than 7 days will incur a flat 10% administrative fee. This fee covers the additional resources required to manage overdue accounts.

Suspension of Service: Players with outstanding balances may be moved to “inactive” status and restricted from practices, games, or training until the account is settled.
4. Financial Aid & Scholarship Policy
212 LACROSSE Financial Aid & Scholarship Policy | All Programs
At 212 Lacrosse, we are committed to ensuring that elite-level training is accessible to dedicated athletes, regardless of their financial circumstances. Our Financial Aid Fund is strictly reserved for families experiencing verified economic hardship. This policy exists to protect the integrity of that fund and ensure that every dollar of community-supported aid reaches a family that genuinely needs it.
I. Eligibility & Philosophy
Financial aid is allocated based on a comprehensive review of a family’s overall financial picture, including, but not limited to, annual household income from all sources, liquid assets, retirement and investment accounts, real estate holdings, vehicle ownership, and lifestyle expenditures. Aid is distributed on a sliding scale and is not guaranteed for any applicant. Scholarship funds are limited and are drawn from program revenues and community contributions.
To be considered for any level of tuition reduction, the applicant must demonstrate that the program fee represents a genuine and prohibitive financial burden relative to the household’s total resources. Families whose documented household income exceeds 300% of the federal poverty level for their household size will generally not qualify, absent extraordinary and documented circumstances.
II. Mandatory Documentation Requirements
To be considered for a scholarship or tuition reduction, applicants must submit a complete Financial Disclosure Packet to mike@212lacrosse.com. Incomplete, unsigned, or partially submitted applications will not be reviewed. The required documents are:
- Federal Tax Returns: A signed, certified copy of the most recent Federal Form 1040, including all schedules (specifically Schedule A, C, and E, where applicable). Tax returns must be submitted in a readable, unprotected format. Password-protected, redacted, or otherwise inaccessible tax documents will not be accepted and will be treated as missing documentation.
- Current Earnings Verification: Copies of the two most recent pay stubs or W-2/1099 statements for all employed adults in the household. Pay stubs must clearly identify the employer, gross and net pay, and all deposit accounts receiving funds.
- Bank Account Disclosure: A current statement (most recent 30 days) for every checking, savings, and brokerage account held by any adult member of the household. Applicants may not selectively disclose accounts. All accounts receiving direct deposits, investment income, or regular transfers must be included.
- Real Estate Disclosure: Applicants must provide the address of their primary residence and a full disclosure of any additional real estate holdings, including vacation homes, investment properties, or land. If renting, a copy of the current lease agreement is required.
- Asset Disclosure: A complete list of vehicles owned or leased (make, model, year), retirement account balances (401k, IRA, Roth, HSA), and any other significant assets, including business interests, trust holdings, or equity stakes.
- Hardship Narrative: A brief written statement (minimum one paragraph) outlining the specific unforeseen circumstances (e.g., involuntary job loss, significant medical expenses, family emergency) that make the program tuition a prohibitive expense for the household.
Verification: 212 Lacrosse reserves the right to cross-reference all submitted information with public records, including county assessor databases, professional networking profiles, corporate filings, and other publicly available data sources, to ensure that community-funded aid is directed to those with the highest level of need.
III. Sworn Financial Aid Application
In addition to the documents listed above, all applicants must complete and sign the 212 Lacrosse Financial Aid Application in its entirety. This is a separate document that requires disclosure of household income, assets, real estate, business interests, trust holdings, vehicles, and monthly expenditures, accompanied by a sworn certification of accuracy.
The Financial Aid Application is a mandatory component of the review process. Submitting supplemental financial documents does not substitute for the completed and signed application. Applications returned blank, unsigned, or incomplete will result in automatic denial of the financial aid request. No exceptions.
The sworn certification exists to protect both 212 Lacrosse and the applicant. It establishes that all information provided is truthful and complete, and it creates a binding record of the household’s financial representations. Refusal to sign the application will be interpreted as an unwillingness to certify the accuracy of the information provided.
IV. Roster Holds During Financial Aid Review
When a family submits a financial aid request for a specific program, 212 Lacrosse may, at its sole discretion, place a conditional hold on a roster spot for the applicant’s child while the aid review is pending. This hold is not a guarantee of enrollment. It is a courtesy extended in good faith to allow the review process to be completed before the roster fills.
The conditional roster hold is subject to the following terms:
- Duration: The hold remains active only during the active aid review period (see Section V for timelines). If the review results in a denial, the hold is immediately released, and the spot becomes available to other families.
- No registration rights: A conditional hold does not grant the applicant any right to register, enroll, or claim a roster spot. Registration may only occur after a financial aid determination has been issued and accepted by both parties.
- Spot availability: If the financial aid request is denied and the roster has since reached capacity, 212 Lacrosse is under no obligation to create an additional spot. The applicant may be offered enrollment in an alternative program or a full refund of any payments made.
- No circumvention: If a financial aid request is pending or has been denied, the applicant may not bypass the aid review process by registering through the public website or any other enrollment channel. Any registration submitted after a denial has been issued will be treated as unauthorized and will be reversed, with a full refund issued to the original payment method. Public-facing registration systems are administrative tools and do not override program decisions.
V. Timeline & Deadlines
- Submission Deadline: All financial aid documentation must be submitted at least two months (60 days) prior to the program start date. Late submissions will only be considered if roster spots and aid funds remain available.
- Review Period: The review committee will evaluate complete applications within 14 business days of receipt. Incomplete applications will not enter review until all required materials are received.
- Notification: Applicants will be notified of the committee’s decision via email. The notification will include the determination (approved, partially approved, or denied) and, if applicable, the amount of aid awarded.
- Acceptance Window: Applicants who receive a financial aid award must confirm acceptance and complete enrollment within 7 business days of notification. Failure to respond within this window will result in forfeiture of the award and release of the conditional roster hold.
- Appeal: Denied applicants who believe their circumstances were not fully reflected in their initial submission may submit a complete, signed Financial Aid Application with any additional documentation within 14 days of the denial. Appeals based solely on disagreement with the committee’s assessment, without new material information, will not be reviewed.
VI. Certification of Accuracy & Consequences of Misrepresentation
By submitting a request for financial aid, parents and legal guardians certify that:
- The household lacks the liquid assets or discretionary funds to cover the program’s cost without material financial hardship.
- All documents submitted are authentic, unaltered, complete, and presented in a readable format.
- No material financial information has been omitted, concealed, or misrepresented, including but not limited to: income sources, bank accounts, assets, real estate, spousal or co-resident income, or financial support received from any other party.
- The applicant has not received, and is not currently receiving, financial aid or scholarship funds from other youth sports organizations for which the household would not qualify under this policy.
Any misrepresentation of financial status — whether by omission, concealment, submission of inaccessible documents, or affirmative false statement — will result in:
- Immediate forfeiture of the player’s roster spot without refund of any tuition paid.
- Permanent disqualification of the family from all future 212 Lacrosse financial aid programs.
- Potential removal from the 212 Lacrosse program entirely, at the sole discretion of the program director.
- Notification: 212 Lacrosse reserves the right to notify affiliated organizations (including but not limited to partner clubs and league administrators) if it determines that a family has engaged in a pattern of fraudulent scholarship-seeking across multiple programs.
212 Lacrosse takes the integrity of its financial aid fund seriously. These funds come from program revenues and community support. Every dollar awarded to a family that does not qualify is a dollar taken from a family that does. We will protect the fund accordingly.
VII. Communication Policy During Aid Review
All communication regarding financial aid applications must be conducted in writing via email. 212 Lacrosse will not conduct financial aid discussions by phone, text message, or in person. This policy exists to ensure that a complete, accurate, and timestamped record of all communications is maintained to protect both parties.
Applicants are expected to direct all financial aid correspondence to the program director. Contacting other 212 Lacrosse staff, coaches, board members, or affiliated organization leadership regarding a pending or denied financial aid application is not appropriate and will not influence the outcome of the review.
VIII. Privacy & Confidentiality
All financial information submitted as part of a financial aid application is treated as confidential and will be reviewed only by the 212 Lacrosse financial aid review committee. Documents are stored securely and are not shared with coaches, other families, or third parties except as required by law or as described in Section VI regarding notification of affiliated organizations in cases of documented misrepresentation.
IX. Policy Amendments
This policy may be updated at any time at the discretion of the 212 Lacrosse program director. The most current version will be made available upon request and will govern all financial aid applications submitted after its effective date.
5. Training Schedules
Schedules are subject to modification based on Basin Recreation facility availability or organizational requirements. 212 Lacrosse will communicate any changes as promptly as possible.
